hmm, you could be right. I just asked the people living here in colombia what they would say if they had a sore throat. So it may be a more colloquial phrase – both phrases may be.

That translation is incorrect, that means “it itches”. The correct translation would be. “Tengo la garganta irritada”. So “tengo piquiÃ±a” means ’something’ itches, and the action would be to scratch rather than to take a medicine.
